Advance Care Planning: It’s About the Conversation!, April 29th at 6:30 p.m.

What if a sudden illness or injury left you unable to speak for yourself?

Who would you want to speak for you?

What would you want them to know about your values and wishes?

This program will provide thoughtful conversation to help you make your wishes known. Learn vocabulary, reflect on your values, choose a healthcare agent, explore goals for treatment, and take the first steps toward completing an advance directive.  Participants will learn about the legal planning tools that can be put in place before an incapacity and the consequences of not planning ahead.  

This event is part of a month-long series of events on Advance Care Planning promoting National Healthcare Decisions Day (https://www.nhdd.org/).  Advance Care Planning is the process of documenting future health care decisions. Topics include having the conversation, pros, and cons of life-sustaining technologies, and understanding Advance Directive forms.
